Requesting Transcripts and Records for MCHS Graduates/Alumni

High School Transcripts:

Graduates/Alumni of MCHS can order their high school transcripts through Parchment.com

SBVC Transcripts:

SBVC Transcripts can be ordered either by:

• Phone: 909-384-4401 or
• In Person: Admissions AD/SS-100, click HERE for in-person availability

Please note: There may be a fee associated with transcript requests, so please be prepared to pay any applicable fees.

MCHS Official/Unofficial Transcripts: 

If you need an official transcript during the current school year (not the end-of-year transcript), please submit a request through the MCHS Official Transcript Request Form

Unofficial transcripts can be printed by the high school counselor or on Thursdays by the registrar

Transcripts can also be requested through the Parchment website. To request transcripts through Parchment, visit Parchment.com

So that you know, the processing times for official transcript requests may vary. Please be sure to submit your request well ahead of any deadlines.

Requesting General Transcripts and Records at MCHS

For all general information or questions email our Registar, Guadalupe Martinez

• Schools and Agencies: Email or fax an official transcript/records request form on school letterhead to our registrar, Guadalupe Martinez, or 909-888-4548
• In your records request, please include the following information:
  • Full legal name of the Student (including any former names)
  • Student's Date of Birth
  • Last year the student attended MCHS
  • Specific records you are requesting
• Parents: Please email our registrar, by clicking her name found above, for further instructions on how to request transcripts or records

Additional Information:

• Diplomas: Students will receive ONE high school diploma upon graduation. Lost or damaged diplomas cannot be replaced.
• Fees: There may be a fee associated with transcript and record requests.
